Submission Window: September 1 through September 30 (first semester) or January 1 through January 30 (second semester) This Google form and a recent monthly statement including the payment stub (many companies provide a pdf copy of the statement) and copy of the provider's W9 form (required). An internet credit of up to $250 per semester per family will be split evenly between all enrolled children of a household is available to be paid directly to the family's internet provider.
This Google form and a corresponding invoice and copy of the provider's W9 form (required) may be submitted for Academic Enrichment classes that occur between September 1 and May 31 of the current school year.
