How to Submit an Order Form
- Select the appropriate request form from below.
- Click the blue "Make a Copy" button.
- Rename the form as "Last Name, First Name Request."
- Fill in all of the appropriate information.
- Select share in the upper right corner and enter in the email for your child's Advisory Teacher and the Odyssey Office. Be sure they have editing rights.
- Monitor the bottom of the form to see the status of your request.

Ordering Forms
Curriculum and Supply Request Form- Ordering Window: November 1 though February 15
There is a $100 limit per student for general supplies which will be deducted from the student's overall allotment; please plan accordingly. Complete one form for each student and share with your child's Advisory Teacher. Please view the Approved Vendor List while ordering items.
Internet Payment Request Form- Ordering Window: September 1 through September 30 (first semester) or January 1 through January 30 (second semester)
An internet credit of up to $250 per semester per family is available to be paid directly to the family's internet provider. Please view the form for additional guidelines.
Lesson Request Form- Ordering Window: October 1 through January 30
This form and a corresponding invoice and copy of the provider's W9 form (required) may be submitted for Student Lessons that occur between September 1 and May 31 of the current school year. Please note that only 60% of the students total allotment may be used for lessons/athletic/extracurricular activities.
Athletic Membership Request Form- Ordering Window: October 1 through January 30
This form and a corresponding invoice and copy of the provider's W9 form (required) may be submitted for Athletic Memberships that occur between September 1 and May 31 of the current school year. (Memberships may not include additional guests who are not family members). Please note that only 60% of the students total allotment may be used for lessons/athletics/extracurricular activities.
Membership Request Form- Ordering Window: September 1 through September 30
Odyssey provides unique educational opportunities for enrolled students and their families in a virtual setting by offering up to two memberships at a facility such as a zoo, museum, or athletic facility-see above form. (Memberships may not include additional guests who are not family members). Please note that only 60% of the students total allotment may be used for lessons/athletics/extracurricular activities.
Item Return/Refund Request Form
If an item needs to be returned for any reason (damaged, wrong item, defect, etc) this form must be completed to initiate the return process.
Invoicing Payment Information & W-9 Form
All organizations or lesson providers receiving payment on behalf of students from Odyssey Academy are required to submit a copy of their W-9 form.
