How to Submit an Order Form
- Select the request form below.
- Click the blue "Make a Copy" button.
- Rename the form as "Student Last Name, Student First Name Request."
- Fill in all of the appropriate information.
- Select share in the upper right corner and enter in the email for your child's Advisory Teacher. Be sure they have editing rights.
- Monitor the bottom of the form to see the status of your request.
Curriculum and Supply Request Form- Ordering Window: November 1 though February 15
There is a $100 limit per student for general supplies and a $200 limit per student for art supplies which will be deducted from the student's overall allotment; please plan accordingly. Complete one form for each student and share with your child's Advisory Teacher. Please view the Approved Vendor List while ordering items.
